This is on the 3rd floor of the General Centre, in the "E"
wing.
Take the central elevators in the
General Centre to the third floor. As you exit the elevators,
our department is to your
left at the end of the hall.

Appointments
All appointments to ultrasound are
made by referral from your doctor. A completed requisition must be
received prior to the appointment.
Please make every effort to keep your
appointment. If you must cancel, please give 24 hours notice
(787-3076) voicemail.
The appointment schedule is very full.
If you are late for
your appointment, or unprepared, your test will be rescheduled for a later date.

| Please do not bring young children
with you. The department is very busy and we do not have
child care. If you must bring children, please bring another
person to supervise them. |
| For patient confidentiality,
please remain in the waiting room until you are called. |

Baby Pictures |
 |
Patients having an obstetrical exam
may request a copy of their images. There is a fee for this service. A Request
to Access Personal Health Information Form must be completed and
signed. The form is available at Ultrasound Reception.
The fee and the completed form must be submitted to the HSC cashier
at
the Sherbrook entrance. The images will be mailed out within 30
days. No refunds are made once the pictures have been
printed.
Personal photographic or recording
devices are NOT permitted in the scan rooms. |
