The Patient Safety Team focuses on inspiring, facilitating and supporting the staff and leadership of HSC Winnipeg to achieve ever-improving safe, high quality care for all patients. We are dedicated to promoting a culture of safety where we encourage individuals to report healthcare errors when they happen, so that we can develop new ways to improve patient safety and provide the best possible care.

What is a Critical Incident (CI)?

A Critical Incident (CI) is an unintended event that occurs when health services provided to a patient result in a serious and undesired consequence such as death, disability, injury or harm unplanned admission to hospital or unusual extension of a hospital stay.

To be considered a CI, the event is one that does not result from the patient’s underlying health condition or a risk inherent in providing the health services (e.g. known complications of a surgery)

Reporting a Critical Incident

Anyone who becomes aware of a Critical Incident is encouraged to report it. Please call the Winnipeg Regional Health Authority’s Critical Incident Reporting Line at 204.788.8222
